Leadership - the plight of the followers

How do leaders "get away" with making shockingly poor decisions, and display what at its most simple level, is self indulgent bad behaviour? What do others charged with leadership at lower levels, think and do about containing such behaviour? What are the responsibilities of board members and external advisors when they are aware that poor judgement at the top is rife?

This article suggests that those who appoint leaders will have to make clear some very different expectations of leaders and those who are followers, will have to manage themselves and their expectations very differently if we are to avoid the unspoken collusion which enables the leaders' excesses to go unchecked.
